2013 Fall Team Application Form

Please fill out this form as detailed as possible. It will be sent to the Commissioner for placement into the Utah MSBL.

Age requirements are:

18+ Divsion ~ players must turn 18 by 12/31/2013

25+ Division ~ players must turn 25 by 12/31/2013

35+ Division ~ players must turn 35 by 12/31/2013
(Father/Son and Grandfather exemption only applies to the 35+ Division)

45+ Division ~ players must turn 45 by 12/31/2013
(45+ teams can have up to 3 players that are 42+. These players are Eligible to play any position)

Games for the fall season are played on Tuesday and Thursday nights, along with Saturdays and Sundays. Teams will play one weeknight game and one weekend game.
