This form should be completed by parent(s) of particpants in the 3rd-5th grade recreation league. Please complete one form for each child in the program. Responses are anonymous, you will only be asked to identify the team that your child played on. A summary of results will be reviewed by WBA Board members in a public meeting. Comments and ratings for coaches will be reviewed by a small coaching selection committee if the coach submits an application to coach in the future. If you have specific question or concerns that you would like addressed personally, please send those directly to wilsonvillebasketball@yahoo.com.
