Collect signed registration forms for every player on team. ( If you are making copies from Web page or original form, you must provide us with 3 copies per player)

It is your responsibility to inform players not returning to your team to register as an individual or find another team.

Collect checks made out to you, or cash from players, and total amount due for your team. Make ONE check in the amount of entire team payable to CPB and attach to team roster form.

Fill out team roster form and make a copy for yourself.

Collect Birth Certificates for all players that have not previously played in CPB and attach to white copy of registration form for each player. If you intend to play tournament ball outside of CPB it is suggested you get a copy of each players B/C and keep for your records.

Separate white copy of registration form from yellow copy and attach team roster form, white copies, (or one copy if making duplicates) with birth certificates, and ONE check made payable to CPB, and bring to registration BEFORE the registration deadline. Keep the yellow copy as it your medical release form for each player.
