Must have completed Rosters, Health Form/ Certificate dated January 1, 2015 or later, Birth Certificate & copy of June 2015 report card with average calculated and highlighted on copy. If below 70, must have Scholastic Eligibility Form completed, if 96 or above should have All-American Application completed. In addition, copies of the certification of the coaches on roster.
