ATTENTION: For All Football Registrations, Parents MUST Bring Child to Registration So They Can Be Weighed In.
FORMS: Forms are available on our website. Please print the Medical Release Form and have this filled out by your physician and bring with you to registration. The Medical Form MUST be dated 2015. Please bring an original Birth Certificate.
