Tiny Mites: Cheer 8:00am/ Football 8:15am
Cadets: Cheer 8:30am/ Football 8:45am
Jr. Pee Wee: Cheer 9:00am/Football 9:15am
Pee Wee: Cheer 9:30am/ Football 9:45am
Jr. Midget/Midgets: Cheer 10:00am/ Football 10:15am
ALL below items are DUE at Helmet Handout on July 16, 2016
1.) Most recent report card (end of 2016 school year, no copies)
2.) Original Birth Certificate (no copies)
3.) Medical Clearance/Physical on league form-
Form downloaded from your account at Sports Illustrated. (Must be original, signed by physician w/ office address stamp,2016 date required, No fax/copies or white out accepted)
4.) All accounts paid in full-
If you wish to pay your balance via credit card, please go to your account at Sports Illustrated prior to July 16th. Cash or check payments only will be accepted at Helmet Handout.
NO EXCEPTIONS!!! Players who have not turned in all above items or whose account is not paid in full, will NOT receive Helmets/Equipment (football) or be allowed to participate in Conditioning Week. Once documents are turned in and/or account is paid in full your child will be allowed to start practicing. Any hours missed during conditioning week will delay your child's certification as those hours must be made up. All players must complete required conditioning hours to be certified.
