AAUA APPLICATION FORM
This form is to be used for new member applications to AAUA. Any applications will be used to provide specific information for new members and for potential new members interested in signing up for the annual training and for the mechanincs clinic. The information received will be kept confidential with the AAUA Board of Directors and the training instructors.

This form is to be used to file a report with the Anne Arundel Umpire Association. This form is transmitted electronically to the Vice President of the Association for his review and actions.
Copies are also sent electronically to the President and Scheduler for information purposes only.

This form is intended to be used by our association members for the purposes of evaluations of each other's perfomances, however individuals are welcome to submit a report.

COACH EVALUATION FORM FOR UMPIRES
This form is to be used for the Evaluation by an individual coach for the overall performance of an individual umpire. Any comments and/or evaluations received will be kept confidential with the AAUA Board of Directors and/or the appointed Evaluation Committee. This form is intended to be used by the leagues we serve to provide input regarding the quality of service received.
