Raffle Ticket Agreement for 2009 Reunion
This form is an agreement between the signee and the Caldwell Reunion. By signing this online form, you agree to the terms and conditions of accepting raffle tickets to sell strictly for fundraising purposes. Tickets must be sold and all unsold tickets must be returned to the Caldwell Reunion, by deadline set. If signee fails to return tickets, signee is responsible for all tickets not turned in and responsible for paying the Caldwell Reunion for unsold tickets. By submitting this form online you are submitting an electronic signature acknowledging you accept the terms and conditions set forth.

2009 Reunion Survey
This survey is sent out at the end of the reunion to discuss any issues or praise our family may have. You can let us know what you enjoyed and what you did not enjoy.
