2010 Fall Ball Registration (click here!)
*Please read the information on the Fall Ball page before you register so you understand what Fall Ball involves!!!
* Forms not submitted and printed properly, OR FILLED OUT IN ALL CAPS, will NOT be accepted, so please follow the instructions. Printing the form at the right time is tricky.
*DO NOT PRINT OUT A BLANK FORM AND FILL IN BY HAND - IT WILL NOT BE ACCEPTED!!!!
*When you have completed the on-line form, please print it out. The option to print will be at the top of the "Completed Online Forms Summary" page, right after you have pressed the "SUBMIT FORM" button. It is not obvious, so look carefully! You must print the form BEFORE you click the "COMPLETE CHECK-OUT" button!!
If you miss printing the form at the right time, DO NOT SUBMIT A DUPLICATE FORM!!!! Instead, contact Rachel Shalhoob (rachelshalhoob@verizon.net) to get a properly formatted copy of your form.
*Fill in your child's name and sign the Medical Release.
*Mail the form and registration check for $90 made out to GVSLL to:

Your child will be on our list once you have submitted the form on-line, but will not be placed on a team until the signed form and check have been received!!
NO REFUNDS WILL BE GIVEN AFTER THE TEAMS ARE FORMED IN AUGUST

Contact Info Form
If you are a GVSLL player and your contact info changes (address, email, home phone) please let us know so our files are up-to-date. We want to make sure you get the latest info, especially when Regsitration time comes around! You can also use this form to be added to our contact list even if you are not currently a GVSLL player so you'll be on distribution for future information.
