Misconduct Report - Boys Basketball
This form is to be used when:
a) Player receives a technical foul or you have an unusual incident/problem @ your game. (Submit form, no call needed to Section)
b) Coach receives technical foul. (Submit online form, only need to Call Section)
c) Player or coach are ejected from game. Call Section by 9:30am, PRINT this report before clicking SUBMIT button and mail or fax(366.4334) report to Section Office. Keep a copy for your records.
All Reported Misconduct Situations are to: Jack Sexton
When writing report do not:
-Recommend a course of action
-Use threatening terms
-Bring up hearsay or past experiences unless they are directly related to incident
-Speculate, they usually cannot be proven
