TEAM SPONSORSHIP FORM for Businesses
This is to allow those businesses who wish to sponsor a sport, team or activity at Kedron Youth to conveniently do so online..
When you become a KYA sponsor,
your business information will be displayed on our website, and if you have a web page or graphic,
we will link our members to your site and/or display your graphic on our webpage.

please download, print and fill out the form in the 'Registration Form' menu to the left of this page.

KYA - Refund Form
Any and all requests for refunds must be made on a signed 'Refund Form'.
These forms may be submitted via this online form by clicking and completing the red
" KYA - Refund Form ", link above.
Some fields are required for online submission.
After completion of the online form,
click the 'Submit Form' button at the bottom and you will receive an automated email confirmation of your forms submission at the email address you provided.
Please print the email, sign and return it to the P.O. Box above to complete your request for refund.
You may also print this blank form, fill it out and then mail this form to the P.O. Box
KYA is a non-profit organization and as such is bound by certain rules and regulations which must be adhered to.
No refunds will be permitted after the sport season begins.
Please understand that uniforms and other equipment is purchased once we begin to receive registrations for most sports.
Refunds must be approved by the Board of Directors and only in certain circumstances which are designated in the KYA By-Laws.
Coaches, or individual KYA Board Members are not authorized to approve refunds under any circumstances.
If any of your children will be playing in another sport,
please request a 'credit' for your family.
