Club Questionaire 2013
The focal point for this survey is receive feedback from our membership in order to continue supporting the children of Plainview Old Bethpage and to provide them with a high quality soccer experience.
Only with your help can we continue to improve and grow, so please take a few short minutes to offer your opinions.
We are allowing each of you the option to supply us with your name, as nothing will ever be held against anyone for their suggestions and opinions. We truly do hope that you do include your information as this would lend a greater sense of validity to this survey.
Please have completed by April 1st, 2013.
Thank you for your participation, and with every 50 survey's received (with a name attached) we will select one questionnaire at random (providing there is a name attached) and provide them with a POBSC Hoodie.

Spring 2013 Practice Field Request Form - Lights Only
In the Spring we anticipate having lighted fields available for weekday & evening practices at the following locations:
POBSC Travel Fields - The Large scrimmage field will have lights with sessions scheduled for (5:00-6:20, 6:20-7:40, 7:40-9).
In order to maximize times and field space, sessions will be 80 minutes in length. The large scrimmage will be divided into thirds. Each team will have approximately a 50x25 yard area.

Spring 2013 Practice Field Request Form - NO Lights
Please use this form to submit your weekly practice/training schedule for the Spring 2013 season for NON-LIGHTED FIELDS ONLY.
Before submitting, please make sure that you speak with your trainer regarding availability.
Please use this form for any changes within the season.
There is limited space available at the travel fields on the weekends. Also note that we share fields with other community based programs.
You will receive a confirmation email if your times were approved. Please allow up to 72 hours for a response during the season.
Initial Spring Schedule will be released week of March 4.
