2010 Returning Player Registration Form
To register for the upcoming 2010 PRGSL season complete the above "2010 New Player Registration" form.
THIS IS ONLY FOR RETURNING PLAYERS from the 2009 season.
If you are a New PRGSL player, fill out the New Player Registration Form. We will accept registration payments at Centennial Park
Saturday, January 9th and 16th from 9:00 a.m. to 12:00 noon.
Bring a copy of this form with payment
The Online Registration fee is $128 with $40 of that fee going towards your participation in the fundraiser. The box of bars you are paying for online, can be picked up at your practices. The fundraiser can be sold to recoup your $40 or you can simply choose to "Opt Out" of the fundraiser for $25 by paying $113 instead of $128, however you will not recieve the fundraiser.

2010 Volunteer Form
Description of Volunteer Positions
Coach - The coach of the team is selected by the Board from a pool of volunteers. The coach will be responsible for notifying selected players, scheduling of practice time, scheduling initial team meeting, reporting of scores and statistics onto the website and pick-up/return of all assigned equipment.
Assistant Coach(s) – Will be selected by the Coach and will assist with practices and games and possibly the reporting of scores and statistics onto the website
Umpire – Umpires will be under the direction of Chuck Hoops and will need to fill out this form and contact Chuck at UIC@PRGSL.org
Team Parent – Will be responsible for any duties assigned by the Coach including, but not limited to, scheduling of snacks, notification of changes in schedules and coordinating team party.
