Derek Pieper Tournament Player Registration Form
All players who are participating in the Derek Pieper Memorial Cup Tournament(November 21 &22)in Wesley Chapel, Florida, are required to complete this registration form. THE DEADLINE TO REGISTER FOR THIS WAS NOVEMBER 11. Registrations received after November 11 will be charged a $30 late fee.
