These forms allow participants to reserve their spot(s) for class. Please submit a Reservation Form for each participant. This online form does not replace the Official Registration Form.
This site is not set up to accept online payments.
Due to the nature of the program you will be notified in the summary that your cost is $0.00. Just to clarify, this class is not free. The site is notifying you that you will not be charged on the website. You will need to mail or hand deliver your registration fee. To see the current cost of classes, click on Flyers under the main menu.

1st Timer Reservation Form

If you have participated in the past, but not since January 1st of this year, please complete this reservation form for first time participants.
Upon completion of the Reservation Form, please complete an Official Registration Form for each participant. This form can be obtained under the Main Menu "Registration Form"
