This form will be used for registration to all basketball tournaments. All information must be completed in detail so that your teams application will not be held out of the bracketing process.

This application must be used for the entry into the SJBBA (Spring/Summer/Fall)basketball leagues. Special high school requirrements are met to assure the Quality assurance of the teams as well as players.
