In School Activity Interferement Form
This form is to be used by a manager in the event of a school activity prohibits a team to field the appropriate number of players (9) to play a scheduled game, This form must be filled out and submitted at least 24 hours prior to the scheduled game in conflict or Rule 4.15 will be in effect.
***PLEASE NOTE***
The system time stamps ALL forms as they are submitted. Be sure you submit the form at least 24 hours prior. Forms submitted even 1 minute late will not be accepted as on time.
