WMYLC Game Reporting Form
This form is for teams in the WMYLC and is used to submit game results to the conference. Please use this form as often as you like.
1. Home team coaches are responsible for completing this online game report for each home game.
2. At each home game field site, make sure to get the officials name and have them sign the scorebook. Since there may be many officials from many different officials associations, it is important to get the officials to sign in as proof of their attendance at your game.
3. After the game, complete this online form. Key things that MUST be reported:
- Teams participating
- Game final score
- Location, date, time of the game
- Any unusual occurrences
- Performance and behavior of the officials
- Behavior of the other coach
- Any game delays due to spectator behavior
- All Unsportsmanlike conduct penalties
- Any ejections
- Any fights
- Any weather, or other game suspension occurrences

WMLOA Officials Request
This form is intended for Club Coordinators to request officials from WMLOA for makeup or add-on games that they have a need to play.
1. YOU MUST MAKE SURE THAT THE FIELD YOU WILL PLAY ON IS AVAILABLE AT THE DATE AND TIME YOU REQUEST. WMYLC is NOT responsible for checking the availability of your fields.
2. An email will be sent to WMLOA with your request and they will contact you back either via phone or email.

Refund Request for No-Show Officials
This form is intended for Club Coordinators to request a refund for games that were played and a WMLOA official did not show up to officiate the game.
1. WMYLC will refund your league fee money upon verification by WMLOA that an official did not appear.
2. An email will be sent to both the Treasurer of WMYLC and to WMLOA with your request. WMLOA takes the information seriously so please be ACCURATE.
