WMYLC Game Reporting Form
This form is for teams in the WMYLC and is used to submit game results to the conference. Please use this form as often as you like.
1. Home team coaches are responsible for completing a game report for each home game.
2. At each home game field site, have a copy of the report form for that game available for the officials to sign in at your home game. Since there may be many officials from many different officials associations, it is important to get the officials to sign in as proof of their attendance at your game.
3. After the game, complete the remainder of the form. Key things that MUST be reported:
- Teams participating
- Game final score
- Location, date, time of the game
- Any unusual occurrences
- Performance and behavior of the officials
- Behavior of the other coach
- Any game delays due to spectator behavior
- All Unsportsmanlike conduct penalties
- Any ejections
- Any fights
- Any weather, or other game suspension occurrences
