I know these are tough to answer without seeing.
Runner on 2nd.
Pitcher is in stretch, goes into set position.
Right handed pitcher.
Pitcher lifts left leg and spins on right heel and throws to second.
He did move his right foot, left foot started movement.
Hard to describe hands, but looked like he was starting a pitch.

Is this a balk?
I thought the pitcher had to disengage from rubber to throw to 2nd.

Without seeing it you are correct that it would be very tough to rule based on what you described. What I can tell you with 100% certainty that a pitcher NEVER has to disengage from the rubber to throw to ANY base. As a coach I always ensured my pitchers stayed on the rubber to attempt a pick off because they are protected by it if they do. Meaning if you stay on the rubber and throw the ball out of play it's a 1 base award, if you however disengage then throw out of play it's a 2 base award as you are now considered an infielder.
