This form must be completed and signed by players, coaches, and parents.

Sponsor Letter
This is the cover letter for the sponsor form and can be sent to potential sponsors as well.

Sponsorship Level Form
Sponsors complete this form and return. Explains different levels of sponsorship. Should be included in the sponsorship packet along with the sponsorship letter and a copy of the Knights Code of Conduct.
