Knights Code of Conduct 2009
This form must be completed and signed by players, coaches, and parents.

2009 Player Application
All potential players must complete and submit with $50 nonrefundable registration fee and $50 nonrefundable deposit--deposit will be applied towards program fees.

The registration fee will be waived to all players who submit completed applications and deposits by October 28, 2008.

Sponsor Agreement
Sponsors complete this form and return. Explains different levels of sponsorship.

Knights Sponsor Letter
This is the cover letter for the sponsor form and can be sent to potential sponsors as well.

Medical Release Form
This form will be kept in dugout with coach at all times. This will expedite medical care in an emergency.
