 Athletic Package
Please fill out the forms in the Athletic package and deliver to the BHS Athletic Director before the start of practice Feb 2, 2009. Students that played fall or winter sports do not need to prepare the package IF the physical exam is still current for the lacrosse season (Feb - May 2009).
