BYL Boys/Girls Equipment Loan Agreement
For parents/players who are borrowing equipment for the spring season, please reserve your boy's or girl's equipment by completing the online registration form. Download the Boys/Girls Equipment Loan Agreement Form and bring it with you when you pick-up your equipment at the designated pick-up date in February. See Calendar of Events.
 BYL Emergency Medical Form
This form must be completed and submitted to the head coach at the first official practice of the season. If you do not have this form completed at the first practice, your son/daughter will not be allowed to practice. No exceptions.

This is BYL's atheletic Code of Conduct for Parents. Each BYL league parent must sign this prior to the season and submit to your team parent or coach.

All BYL League coaches must sign this athletic Coach Code of Conduct Form. Submit to the league administrator prior to the start of the season.
