 Equipment Return Checklist
Please print this out and complete before returning your equipment. A CYFDTA representative will check in your equipment and give you the bottom portion of this form as your receipt.
CYFDTA Medical Release form
Physical Exam form-Print and fill out form where applicable and have Physician sign and fill in designated areas on form and turn into Sectretary. All Tackle Football players must have a physical approving the player to play.
CYFDTA Sponsorship Brochure
This is the Cypress Football & Drill Team Alliance (CYFDTA) Business and Personal sponsorship brochure. CYFDTA requires additional funding in addition to the registration fees to maintain this quality program. The money raised from business (and personal) sponsorships also helps our players offset their registration fees by providing a form of scholarship. The CYFDTA is an IRS recognized 501(3)c corporation and all donations are tax deductable per IRS regulations. Please take time to solicit financial assistance from places where you do business or possibly your employer. Every dollar received from sponsorships goes directly into the program as there are no paid employees for CYFDTA. This is a 100% volunteer effort.
This letter and the corresponding sponsorship letter are designed to be printed on both sides of the paper. You simply print the number of copies you need with "page 1" only. Then insert the paper into your printer to print on the back side and select "page 2" only.

This is the sponsorship letter that goes along with the Sponsorship Brochure. Use this letter to introduce the Cypress Football & Drill Team Alliance (CYFDTA) to potential sponsors. Remember that a portion of the sponsorship money you recruit will offset your registration fees.
