Grievance Form
A Grievance form should be utilized to report serious incidents or issues to the CYFL Board. Before filing a grievance, please try your best to resolve the conflict with those parties involved, by first seeking assistance of your team's Head Coach and/or your town's Area coordinator or Division representative before involving the board.
If you cannot come to a satisfactory resolution, please fill out the attached form completely and mail to us at the following address:

CYFL Accident Insurance Claim Form
CYFL accident medical insurance covers all players and coaches. This insurance is secondary to that of the primary insurance carrier covering your child, unless he/she is not covered by any other insurance. If your child is injured during a CYFL practice or game, please download and fill out the information needed from you on this form completely before taking it to your team's coach.
Once your coach has completed his part, please contact Jim Simpson with First Victoria Insurance Agency at jsimpson.firs235@insuremail.net for help in filing the claim. Please follow the instructions carefully on what information is required in order to process a claim.
