On Saturday, July 12, we will conduct fingerprinting on all coaches, key volunteers, and administrators of the league. Download this form now to save time on that day. The league is paying the fees for you!
Complete the 3rd box. When you type on the first page, the info self populates on the 2nd and 3rd pages automatically. Print the document once, but it will print 3 copies. Bring all with you on July 12th. Fingerprinting will be done in conjunction with equipment handouts at the V.A. Hall.
