This form should be completed by anyone wishing to coach or assistant coach and by all EBA board members on a yearly basis.

This insurance certificate should be carried by the team at all times during practice, games or other team events. In the event a claim must be filed, please call the number listed and a claim form will be mailed to you and must be approved by Steve Hendrix or Danny Smith prior to submitting to the insurance carrier.
PLAYER REGISTRATION CARD 2008
This form should be completed, signed and brought to registration session with payment to register player for baseball or softball.
