YTCIE Member Code of Conduct
For Tackle and Cheer Only: You must complete the Youth Tackle Conference of the Inland Empire Code of Conduct and works in tandem with our Eastvale Code of Conduct to ensure those that participate in our program abide by these rules.
Head Coach Application
All interested Head Coaches, please fill out completely. An interview will be scheduled for all Head Coach applicants.
Volunteer and Coach Background Check Form
All Volunteers, Coaches, AD's, Team Mom, etc... are required to complete this form. This and the JCSD Consent Release Form work in tandem to ensure those that have contact with our participants are approved accordingly.
2009 Mustangs Physical Form
For All Participants: This form must be completed by a qualified Doctor of Medine/Osteopathy, Nurse Practicitioner or Physician's Assistant. This record of Physical Examination must be performed no earlier than May 1, 2009 and prior to the player participating in any physical activity in our program.
