Fall Ball 2009 Application
Just a few requests when filling out the registrations forms:
1. Please enter player's full legal name, and you can put whatever name player prefers to be called in quotes. (ie: "Bobby" instead of Robert).
2. Don't forget to include players actual birth date.
3. Please include legible email address
These 3 requests will make further processing of the players registration more accurate and efficient. Thanks in advance for your cooperation
