 Player Application
Fill out this form on your computer,print it out and send it, along with the correct signup fees, to the address at the bottom of the form. Be sure to make a copy for your records. You can also bring the completed form to the signup booth at Brier Elementary School. Remember to bring with you a copy of the player's birth certificate that we can keep if this is your first year with FNYB. Check the Signup Information handout to determine the fee schedule, division and tryout dates.

Manager/Coach Application
This form must be filled out completely by any potential manager or coach. Turn in your completed application to any member of the FNYB Board of Directors, or drop off at the next FNYB Board meeting. Be sure to read page 2 so you know the responsibilities of a manager or coach, and the goals of our league.
FNYB Website Approval and Authorization Form
Form must be completed and turned in to your Manager prior to the start of the season.
Authorization to Treat a Minor
Form must be completed and turned in to your Manager prior to the start of the season.
FNYB Team Information Form
Information on this form will be used to update the FNYB website with contact information for each team. Managers should complete this form and turn in to their respective Commissioners.
 FNYB Reimbursement Form
Use this form to submit reimbursement claims for FNYB expenses.
FNYB Sponsorship Package
This file contains both the sponsorhip letter and sponsorhip form. Completed forms should be forwarded to the FNYB Sponsor Coordinator.
FNYB Sponsorship Program/Rules
Contains a description of the FNYB Sponsorship Program including specific rules regarding how sponsorship money may be spent.
 Sponsorship Reimbursement Form
This form is to be used by the Business Managers for reimbursement claims against their teams sponsorship funds.
Publicity Form
Use this form to report your game results to the Argus. To report game results for publication in the FNYB Line Up newsletter, simply email the information to the Publicity Coordinator.
