There are NO exceptions to these restrictions. 2009 CAYFL Player Registration Form 2009 CAYFL Code of Conduct Each player and parent needs to sign and turn into business manager. 2009 CAYFL Adult Volunteer Form Any adult wishing to help out during practices, game days, etc. must fill out one of these forms and turn it in to your business manager. 2009 CAYFL Volunteer Forms Must be completed by all volunteers in addition to the Adult Volunteer form. 2009 Medical Consent Form This gives your consent for your player to be treated by emergency personnel if necessary. This is a mandatory form 2009 Medical Clearance Form This is the form to be filled out by the players doctor indicating medical clearance to participate in football. This is mandatory 2009 CAYFL Diamondbacks Night 2009 CAYFL Night at the Diamondbacks. The players will get to walk onto the field and will be recognized during the game.
