FORM 1 of 2: Registration - HFYSL Summer 2009
To be printed, signed, and mailed with Medical Release and your check. Cost is $40 per player. $95 per family of 3 or more players. Additional $5 per child late fee after March 31.

FALL Soccer Registration Form 2009
To be printed, signed, and mailed with your check to register area residents in grades 3, 4, 5 and 6 for the Fall 2009 soccer league. Cost is $50 per player. $120 per family of 3 or more players. Please make checks payable to: Honeoye Falls Soccer League and mail to: c/o Kathy Palmer, 168 Victor-Mendon Road Mendon, NY 14506. Registration deadline is June 30, 2009. Late registrations will be placed on a waiting list.
