FOR the 2010 SEASON: This form must be SIGNED & DATED by physician AFTER January 1, 2010. May submit any time after January 1, 2010 through paperwork deadline. Exam MUST have been between January 2009 and June 30, 2010. Please schedule exams prior to paperwork deadline! ALL 2009 exams are acceptable, but this form MUST be printed, signed & dated by physician AFTER January 1, 2010 to be accepted. NO 2008 exams will be accepted!
