US Lacrosse Code of Conduct - Please Read You must read and agree to this code of conduct to be a member of the Hunterdon Lacrosse Club and US Lacrosse. The registration form requires we abide by this code. 2010 HLC Registration Form This form is to be used for all new players who do NOT have siblings in the HLC living in the same household. 2010 HLC Registration Short Form This form is to be used for existing players and siblings new to the HLC and living in the same household ONLY.
