 2013 Volunteer Application
The attached is the 2013 Volunteer Application. This is mandatory for all head coaches, assistant coaches & team parents (team moms) for this season.
If you are applying to be a head coach, please review and complete this form by no later than Thursday, Feb. 7th.
Send the completed application to John Frantz (john.frantz@havasimpact.com) and Ashley Rogers (jr.cougar.president@gmail.com) by the date noted. Make sure you send to both parties.
If you are applying to be an assistant coach, please review and complete this form by no later than Friday, Mar. 1st. Send the completed applications to John and Ashley (as noted above). Once head coaches are selected, these applications will be shared with head coaches for review and selection.

Jr. Cougar Physical Form
Must be completed by a physician and turned in at equipment pick-up, prior to receiving equipment. Player is ineligible to play if the physical form is not on file. All pages must be completed and submitted. Physical MUST be dated on or after January 1, 2013.
