 2009 Pop Warner Volunteer Application
This form is the official 2009 Volunteer Application Form. This form must be submitted before final appointment is made to a volunteer position.
To expedite the appointment process, we accept the on-line Mt. Baldy Volunteer application form pending the start of the season.
 2009 LVSD Policy Form
This form must be read, signed and turned in with a copy at time of registration, along with the 2008 Participant Contract.
 2009 Participant Contract
This is the Player/Cheer Registration Form. Please print and complete form. Bring the original and a copy of both pages to sign-ups.
Please ensure that all fields are completed. If the field is not applicable, write "N/A".
This contract must be completed by the child's Legal Parent/Guardian. If child has a legal court appointed guardian, court documents must be submitted.
Contract cannot be signed by relative, or friend, unless accompanied by court appointed legal guardian.
NOTE: Contract must be signed by player/cheerleader also.
2009 Physical Form
This document is not turned in until Physical is completed. The first page is completed by the parent/guardian and the second page is completed by the doctor performing the physical.
Please ensure that the document is stamped with the doctor's offical stamp, identifying the facility where physical was performed.
Ask the doctor to complete actual height and weight (check marks are not acceptable).
Once Physical is complete, the original and a copy of both pages must be turned in to Certification.
