2013 SJLAX Score Reporting Sheet
The attached is the score reporting sheet that is to be used at every 5th - 8th league game this year.
The procedures are as follows:
1. Forms must be at scorer's table.
2. Home team is responsible for accurate completion
3. Visiting coach must initial
4. Officials must initial.
5. Original must be kept by home team as supporting documentation in case of a conflict.
