Parent Driver Form
This form is needed for every parent.
For the section ending with "the following students:" write "Newport High School Cheerleaders"
For the section ending with "activity trip to" write "various away high school games and cheerleading events"
For the section ending with "on (Date)" write "2008-2009 School Year"
Student Driver Form
This form is needed if a cheerleader is going to drive themselves to any local events/practices/games.
For the "Activity" blank, please write "Various Home Games and Local Cheerleading Events"
For the "Date" blank, please write "2008-2009 School Year"
For the "From / To" blank, please write "Start of Game/Event" and "End of Game/Event"
Student Passenger with Parent Driver Form
This form is needed to allow the cheeleader to ride in the car of another parent.
For the "Destination" blank, please write "various high school games and cheerleading events"
For the "Date" blank, please write "2008-2009 School Year"
For the section ending with "for the purpose of" please write "Cheerleading"
For the "Name of Parent" blank, please write "Parents or adult siblings of other Cheerleaders"
For the "Telephone Number" blank, please write "Available on the Cheer website"
Student Passenger with Student Driver Form
This form is needed to allow the cheeleader to ride in the car of another cheerleader to local events/practices/games.
