Mini Camp Liability Release

Uniform Order Form
This is the form that needs to be handed in by April 1st to order you uniform pieces.

Emergency Contact Form
This form must be completed by all athletes prior to being able to participate in any manner.

Parental Permission Form
This form must be filled out by all athletes prior to being able to participate in any manner.

Athletic Waiver
This waiver must be signed and turned in, accompanied by the athletic fee in order for any athlete to participate in any manner.

Sports Physical Form
This form must completed by a doctor on a yearly basis for all athletes to participate.
