Registration Form - All Teams!
Due to the demand, two additional registrations will be held at the Tampa Regional Library, on the following Dates:
Sunday 11/01 12:30pm-2:00pm
Tuesday 11/03 5:30pm-7:30pm
Cost: $125 Boys grades 2-4 and all Girls
$150 Boys Grades 5-12
In an effort to keep registration costs down, we will be requiring that each player sell 5-$10 tickets for a chance to win one of four vacations, including an all-expense paid trip for four, to the 2010 Lacrosse Championships in Baltimore, Md! Other trips for the 'non-lacrosse' friends and families will be offered too! Players will be able to 'raise' some 'funds' of their own, if they want to sell additional tickets!
Medical Form - Physician must fill out!
If you have a physical form from another fall sport, a copy of that would be acceptable.
Commitment Agreement
Commitment agreement to be read and signed by players, parents and coach, prior to the start of the season.
