This letter is to be used for students who are currently enrolled in Kindergarten and didn't attend school the year before.

2013 Volunteer Forms
All board members, coaches, team parents, must complete a background check in order to participate in the 2012 season. Volunteer forms must be approved prior to August 1, 2011. A copy of the individual's driver's license must accompany the form.

2013 Physical Form
ALL Physicals must be completed on the official Pop Warner Physical Form. Physicals MUST be performed after January 1, 2012, and must be an original signature.
2013 Patch Order Form
Patches must be placed on all participants uniforms prior to Jamboree. Please refer to the patch placement form for where they need to be displayed. This is the first year that ALL Football Players MUST HAVE THEIR PATCHES ON THEIR JERSEY.

2013 Birthday Checker
This form is great for verifing the child's age prior to team placements. Remember the birthday cut off is July 31, 2011.
2013 Home School Form
If the participant attends a home school, this form must be completed by a Board Certified Tearcher, and at least 1 evaluation will need to be on file and completed for the child between Sept. 21 and Oct. 21. or the child will not be allowed to participate in the remainder of the season. Additional information regarding scholastics can be found on the scholastics menu.
2013 Scholastic Eligiblity Form
If the child falls below a grade point average of 2.0 in the 2011-2012 school year, or does not have proof of scholastics during this time frame, then this form must be completed. An evaluation from his/her teacher will need to be conducted between September 21 - October 21 and must contain the school's official letterhead, in order for the child to continue to participate. Additional information regarding scholastics can be found under the scholastics menu.
2013 All American Application
This form should be used for those participants who qualify for the All American Scholarship Program. Please reference the Scholastics page for additional information.
If the participant is absent from a game (or book certification) during the season, or will not attend a post season game. This form is required for each absence.
