2009 Volunteer Forms
All board members, coaches, team parents, must complete a background check in order to participate in the 2009 season. Volunteer forms must be approved prior to August 1, 2009. A copy of the individual's driver's license must accompany the form.
2009 Player/Cheer Contract
2009 Physical Form
ALL Physicals must be completed on the official Pop Warner Physical Form. Physicals MUST be performed after January 1, 2009, and must be an original. NO FAXES or Copies are accepted.
2009 Medical Release League Form
2009 Home School Form
If your child attends a home school, you will need this form completed by a Board Certified Tearcher, and at least 1 evaluation will need to be on file and completed for the child between Sept. 16 and Oct. 16. or the child will not be allowed to participate.
2009 Scholastic Eligiblity Form
If the child falls below a grade point average of 2.0 in the 2008-2009 school year, then this form must be completed. An evaluation from his/her teacher will need to be conducted between September 16 - October 16, 2009 in order for the child to continue to participate.
 Participant Waiver Form
2009 Absentee
If the participant is absent from a game during the season, or will not attend a post season game. This form is required for each absence.
 2009 Patch Orderform
10% early discount if order is received on or before July 8th (price $1.35).
$1.50 per patch if order is received July 9th – August 1st.
$2.00 per patch price if order is received on or after August 2nd.
