 Coach Background Check Form
For All Current and Prospective Coaches;
If you have not submitted to a back ground check within the last 2 years you need to fill out a background check authorization form and submit it along with your coaches contract prior to being given a team. Once you have been notifed you have a team use the PYBSA Player Protection From to get signatures from the parents that their child can play on your team. Without this form we will NOT consider a player as protected for your team.

Coaches Contract
All Coaches must fill out this contract and submit to the PYBSA Board of Directors.
 Player Protect Form
This form must be handed in at the draft in order for you to claim a protected player.
