NCSA Match Day Game Form
Official NSCA Match Day game form that is completed by the HOME TEAM coach for each league game. Each team is required to provide TWO copies of their US Club Team Rosters.

ENYYSA State Cup Registration
Teams U11 and higher are eligible to participate in the NY Challenge or NY Open State Cups.
ENYYSA Permission To Travel
Request for a NY team to travel to another state for league (e.g. NCSA) or tournament (e.g. Whitewater Cup) play.
US Club Soccer Staff Background Check
Access to US Club Soccer for Staff Background Check requests. You will register under Counties United Soccer League (CUSL). Registration is required for all coaches, assistants, trainers, and team managers.
US Youth Soccer Risk Management Registration
Access to ENYYSA Risk Management Registration System. You will register under Counties United Soccer League (CUSL). Registration is required for all coaches, assistants, trainers, and team managers.
