 2013 Player Registration Form
As of 3/16/12 we will continue to accept T-Ball Applicants.
If you are interested in regestering for a division other than T-Ball please contact Deb Shaffer before submitting an application.

 Coaches Application
Everyone interested in managing or coaching a team for the RPYBSL must complete the Coaches Application as well as the Coaches Code of Ethics.
Once you complete and sign both pages of the form please submit it to the League as soon as possible. You may submit it at a monthly meeting (First Wed. of each month), via e-mail or mail it.
Medical Release Form
All Team Managers are required to have each player complete a medical release form.
This form is to be carried by any Regular Season or Tournament Team Manager together with team roster.
Parent Code of Conduct
All Team Managers are required to have the Parent or Guardian of each player on their team Review and sign the Parent Code of conduct.
