Registration Form
This form must be completed in it's entirety along with the Code of Conduct for your child to be able to participate in our organization. Incomplete or missing forms will result in dismissal from the squad/team.
Registration fees:
Football
$75 for first child. $50 for each additional child.
$100 post-dated check (dated 11/15/2011)for equipment deposit is required for EACH child. This check will NOT be cashed and returned to you when the equipment is returned at the end of the season. FAILURE TO RETURN EQUIPMENT WILL RESULT IN THE CHECK BEING DEPOSITED!!
Cheerleading
