Referee must be given a roster prior to the start of a game
 Pick Up Player Slip
Leagues that allow pick up players require the opposing coach to sign this form. Pick up players must meet age requirements and have a valid INSA player card. The form must be shown to the referee.
Player Registration Form
This form is also available at the Spokane Valley Youth Soccer Assoc. office. Form must be completed and signed when player card is purchased.
Team Registration Form
All players on a team are listed on this form.
