 Game roster card
Roster card given to referee at the start of each game.
 Pickup Slip
Leagues that allow pick up players require the opposing coach to sign this form. Pick up players must meet age requirements and have a valid INSA player card. The form must be shown to the referee.
Player Registration Form
This form is also available at the Spokane Valley Youth Soccer Assoc. office. Form must be completed and signed when player card is purchased.
Team Registration Form
Each team completes this at the beginning of every season.
