SPONSORSHIP FORM
Dear Prospective Sponsor:
The Southwest Youth Football Association (SWYFA) program is organized as a non-profit 501(c) (3) organization to provide the youth of the Osborne school community and surrounding area the opportunity to participate in an organized youth football and cheer program. SWYFA provides an environment that teaches the fundamentals of the sport programs to youth participants age’s five to twelve, while encouraging academic excellence, physical fitness, sportsmanship and dedication. We believe that there is a high correlation between the development of the skills that are learned through participation in team sports and success later in life.
SWYFA is operated and managed by a group of dedicated volunteers who give their time and efforts to make the program a success. A key reason for the success we realize each year is the strong support of our sponsors. We gratefully acknowledge our sponsor’s role in improving the quality of our program. We appreciate your help in providing the resources for us to improve what we do for our participants.
All costs associated with operating our program are derived from registration fees and donations from community members. All funds go toward the betterment of the program, including equipment, uniforms, field maintenance, referees, etc. Remember, community support is crucial to our success. Your donations are tax deductible and can take the form of monetary or non-monetary contributions
There are a number of ways that you can help. Our primary sponsorship vehicle is the purchase of a sponsorship package. There are five levels of sponsorship, each with their respective benefits. In recognition of your support and generosity, we have designed benefits which allow you to realize due recognition from our members.
Regardless of how you support the Southwest Cardinals, remember that your money will go to help the children of the Osborne school community and the surrounding area.

REGISTRATION FORM
To sign up in person or by mail follow these steps:
1. Download the Registration Form.
2. Fill out the forms with the appropriate information.
3. Sign and date the forms.
4. Review the Registration Information located in the "Handouts" section for important deadlines.
5. Submit the required checks and forms at a registration event or mail the checks and forms to SWYFA:
